Current Customers: How to Submit a Renewal Application

Step 1: Download the appropriate application form(s):  

• Download the Premier Plan Renewal Application by clicking the Download Application button at right >> 
  
  
• Premier Plan Renewal Supplemental Application
  Please Note: The Supplemental Application is a multi-page document that may take longer than usual to download.
  
  
• Free Download of Acrobat Reader (required for downloading PDF files)

Step 2: Complete the application(s):

• The forms are Adobe Acrobat documents that can be completed using Acrobat Reader
  
  
• Click into the appropriate fields and enter your information
  
  
• Print the completed form, as you will not be able to save the changes you have made
  
  
• If you prefer, you may print the blank form and type in or hand-print your information
  

Step 3: Submit your renewal application(s) to Brunswick Companies: 

• Via Email:
  Scan the completed application, as well as any accompanying documents, and email to 
  webleads@brunswickcompanies.com
  
  
• Via Fax:
  Fax the documents to 330.864.8661, Attention: Professional Liability Department
