How to Submit a Premier Plan Application to Brunswick Companies

Step 1: Download the appropriate application form(s):

- Premier Plan New Business Supplemental Application
Please Note: The Supplemental Application is a multi-page document and may take longer than usual to download.
Step 2: Complete the application(s):
The forms are Adobe Acrobat documents that can be completed using Acrobat Reader
Click into the appropriate fields and enter your information
Print the completed form, as you will not be able to save the changes you have made
If you prefer, you may print the blank form and type in or hand-print your information
Step 3: Submit your Premier Plan application(s) to Brunswick Companies:
