How to Submit a CPA EmployerGard EPLI Application to Brunswick Companies

Step 1: Complete the application:

• The form is an Adobe Acrobat document that can be completed using Acrobat Reader
  
  
• Click into the appropriate fields and enter your information:
  
  
• Print the completed form, as you will not be able to save the changes you have made
  
  
• If you prefer, you may print the blank form and type in or hand-print your information
  

Step 2: Submit your License & Permit Bond application and supporting documents to Brunswick Companies:

  Scan the completed application, as well as any accompanying documents, and email to
